I, the undersigned

As a competitor, I will train and prepare as taught by my instructor and will compete with a positive attitude to represent myself, Instructor and school with pride and respect.

 

Liability Waiver

  • undersigned, do hereby voluntarily submit my application for attendance and participation in said Extreme Martial Arts Center® annual tournament, and hereby represent that I am physically fit to participate in said tournament.  I further acknowledge the existence of some risk of personal injury in participating in said tournament, and I hereby assume full responsibility for any and all damages, injuries, and or losses that I may sustain or incur, if any, while attending or participating, and waive all claims against the promoters, or operators, or sponsors of said tournament, individually or otherwise for any claim for injuries I sustain.  I fully understand that any medical treatment given to me will be of a first aid treatment type only.  If under 18, this release and consent is to be signed by parents or legal guardians.

    I/We fully understand and acknowledge that there are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury partial and/or total disability, paralysis and death. I/we voluntarily accept and assume such risk and responsibility for any losses and/or damages following such injury, disability, paralysis or death, however caused and whether caused in whole or in part by negligence of the releases named below. I/We HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE Extreme Martial Arts Center or any of its promoters.

     No Refunds for goods or services, all sales are final.

      We have the right to combine divisions when necessary.
